Multiple small changes

- Costs can now also be a function (e.g. costs depending on premium frequency)
- Additional extraChargeGrossPremium (charge on the gross premium after reserves, but before unit costs)
- Fix bugs due to parameter structure changes (xlsx export broken)
- Modify all valuationTable_* functions to valuationTable.*
- Use roxygen to load dependencies rather than loading explicitly
- Remove debug output
