Add costs with basis NetPremium

• Add basis "NetPremium" to the costs array
• Set up costs cash flows for "NetPremium"
• Include Net Premium in premium calculation coefficients
• Adjust the formula for premium calculation to include costs based on net premium, too
• Document the additional cost item in the .tex file
